Student-Athlete Eligibility Checklist

If you plan on participating on an intercollegiate athletic team at Northampton, you must meet all NJCAA eligibility requirements, which will be outlined by the Director of Athletics. In addition, this is a list of items you will need in order to try-out for a team (please note, more information may be required).

_____ Proof of enrollment in at least 12 credits (full-time) in the current semester
*must remain enrolled full-time throughout the semester

_____Copy of your high school transcript
*must have your name on transcript
*must have a signature of a school official
*must have the high school name on the transcript
*must have your graduation date listed

_____Copy of your high school diploma

_____Official Transcripts from other colleges you have attended (if applicable)

_____Proof of Health Insurance (including a copy of your insurance card)

          *must be on the official Northampton physical form

_____An IMPACT concussion test administered by the athletic trainer

_____Updated shot records

_____Ability to fill in all gaps between your high school graduation date and the current semester (if applicable)
          *this must be detailed, including all dates, employment, locations, etc.
