Student-Athlete Eligibility Checklist
Individuals that plan on participating on an intercollegiate athletic team at Northampton must meet all NJCAA eligibility requirements, which include a student being enrolled in a minimum of 12 credit hours. In addition, all the items/forms listed below are required prior to your first practice.

Items that can be completed/uploaded via Front Rush:

_____Statement Concerning Eligibility
_____Buckley Amendment
_____Birth Certificate
_____Driver’s License
_____Consent for Disclosure of Health Information
_____Health Insurance Information Form
_____Risk Statement

Items that you must bring to athletic office and drop off:
_____Athletic Physical (form available on nccspartans.com)
_____Insurance Card (need copy of front and back of card)
_____Results of a sickle cell test (information available on nccspartans.com)
_____Updated Shot Records
_____Code of Conduct Form (form available on nccspartans.com) Item that can be sent to Northampton Community College:
_____High School Transcript Transcript must include:
             Name of High School
             Student’s Name
             Signature of School Official (or sent via Parchment)
             Graduation Date

Items that will be administered on-campus once you arrive:
_____Impact Concussion Test
_____NJCAA Admin Portal Registration
_____Transfer Paperwork (if applicable)
_____Delay/Break in Attendance Paperwork (if applicable)
_____Course Schedule with Start/End Dates
